Culver City High School & Middle School have streamlined their clearance process.  CCHS & CCMS Athletes must be cleared through  PARENTS create an account for this website to create clearances for all of their children.  (Please do not use student’s ccusd email addresses for account set up) Physical exam forms are to be printed from the site, filled out by your physician and then uploaded to or may be turned in to the school offices.  Should you have any questions, please contact Leslie Gardner 310-842-4200 ext. 3512 or via email at 


July 13, Dr. O'Byrne will be at CCHS to give physical exams for $30 cash to CCHS Athletes at 1:00 pm.
Athletes must sign up with their Head Coach who will give them a parent permission slip to bring on July 13th.
