Please complete the Volunteer Registration Packet below; print, sign and bring all forms along with a copy of
current TB Test results to JoNellia Guinn in the CCHS Principal's office. Paid coaches must also complete the Classified Application packet. You will receive the "CCPD Request for LiveScan" form
only after all paperwork has been completed & returned and payment received for the LiveScan process..($32 for DOJ or $47 for DOJ & FBI) .
